Matt dc075f8969 Fix migration with fully idempotent SQL to handle partial application ...

Rewrote pipeline/stage migration with defensive guards:
- DO blocks with EXCEPTION handlers for enum/constraint creation
- IF NOT EXISTS on all CREATE TABLE/INDEX
- IF EXISTS on all DROP INDEX
- Handles re-running after partial failure
